Underwriting Parameters

A complete submission includes:

• ACORD application, 3 years currently valued loss runs (within 90 days of effective date), and the client’s state tax ID number.
• MVR’s for Transportation Related Accounts
• Average Rate should be greater than $6.00 (total payroll/modified premium/100)
• Supplemental Applications for Major Classes (Manufacturing/Transportation/Contractor)
